CHANGE OF OWNERSHIP INSPECTION FOLLOW-UP. FACILITY TOOK OVER ESTABLISHMENT AND HAD TO GO THROUGH ALL THE OLD PRODUCT AND EQUIPMENT AS STATED IN THE LAST INSPECTION.
FACILITY HAS STRAIGHTEN OUT THE STORE AND A FULL INSPECTION WAS COMPLETED. NO ISSUES WERE NOTED. FACILITY IS GRANTED PERMISSION TO RESUME OPERATION BY THE KANKAKEE COUNTY HEALTH DEPARTMENT. |
